Lightbeam Health, Primary Partners, and Alliance for Community Health Win 2024 SDOH Points of Light Recognition Award from KLAS Research

Recognized by KLAS for addressing social determinants of health (SDOH) and enhancing whole-person care; organizations successfully reduced readmissions for patients with complex needs by 3%

Lightbeam was selected for two Points of Light case studies. This case study focuses on the Primary Partners and AFCH collaboration using Lightbeam’s population health management platform to identify complex-needs patients and their underlying social needs. Using Lightbeam’s comprehensive platform, independent providers across the organization and community-based organization (CBO) could analyze patient data, create care plans, and track interventions at the population and patient level. The initiative resulted in a 3% reduction in preventable readmissions, enhanced patient engagement, and a decrease in mortality rates among engaged patients by half. In total, 301 patients with complex needs improved health outcomes from the outreach program.

The SDOH Points of Light recognition is given to healthcare organizations, payer organization, CBOs, government organizations, and HIT vendors who demonstrate close collaboration that leads to shared outcomes. KLAS validated each submission vis a standard set of questions administered during in-depth interviews with representatives from all relevant stakeholders; these questions asked stakeholders to provide (1) an overview of the engagement, (2) engagement outcomes, (3) best practices and lessons learned, and (4) the overall impact the engagement has in the healthcare industry. Interviews were conducted from May 2024 to July 2024.
